Pediculosis (Head Lice)
No student will be denied an education solely because of head lice
infestation and his educational program shall be restricted to the extent
necessary to eliminate the risk of transmitting the infestation.
It shall be the responsibility of the principal or designee to notify the
parents in the event a child has pediculosis (head lice) and must be picked
up at school. An instructional letter will be sent home to explain the
condition, the treatment, requirements for readmission, deadlines for
satisfactory completion of the treatment and how to access the school nurses
office.
Exclusion Procedure: When nits or lice are noted, the child
will be excluded from school.
Upon exclusion, satisfactory evidence must be submitted to school
personnel that the student has been treated for pediculosis (head lice).
This evidence may include but not be limited to:
- proof of treatment with a pediculicide product (head lice shampoo), and
- satisfactory examination by a school health official.
Re-admission Procedure: After treating and removing all
nits, the parent should bring the child to the office of the Johnson City
School Nurses. A yellow admit form will be given to the parent if the
child is nit and lice free.
A student will be expected to have met all requirements for treatment and
return to school no later than two (2) days following exclusion for head
lice. All days in excess of the allowable period will be marked an
unexcused and referred to the attendance supervisor at the proper time.
Treatment and prevention procedures have been developed by the
school nurses and distributed to each school office (school
personnel/secretary and home school coordinator). Educational videos
and pamphlets are also available for school staff/parents/guardians.
Physical Examinations
The principal shall ensure that there is a complete physical examination
of each student prior to:
- Entering school for the first time. This applies to preschool,
kindergarten, first grade, and other students for whom there is no health
record. Students entering kindergarten or any other pre-school
program must have a physical examination done no earlier than August 1 of
the year preceding the year of enrollment. This must be presented by
the time of entrance.
All other new students and transfer students will have thirty (30) days
after enrollment to present the record of a physical examination by a
physician or other health provider. The physical examination
contained in the records from the student's previous school will be
acceptable; and
- Participation as a member of any athletic team or in any other
strenuous physical activity program.
Cost of the examination shall be borne
by the parent of the student. These records shall be on file in a
secure location.
Immunizations
No student entering school, including those entering preschool,
kindergarten, first grade, those from out-of-state, and those from nonpublic
schools, will be permitted to enroll (or attend) without proof of
immunization, as determined by the Commissioner of Public Health. It is the
responsibility of the parents to have their child(ren) immunized and to
provide such proof to the principal of the school which the student is to
attend.

DTP-DTaP-DT-Td
4 or more doses, one of which must be given on or after the fourth
birthday. If starting series at age seven or older, only three doses are
required.

Polio (OPV/IPV)
4 doses of OPV or IPV. If the 3rd dose was given on or after the fourth
birthday, the 4th dose is not required. However, if a combination of IPV/OPV
(2IPV/2OPV) is used, all four doses are needed regardless of age.

MMR
2 doses given on or after the first birthday required, for entrance to all
grades effective July, 2001. The 2nd dose must be administered at least 30
days after the first dose.

Hepatitis B
3 doses, for entrance into Kindergarten, 1st, 2nd, 3rd, 4th, 7th & 8th
·
Varicella
(Chickenpox)
1 dose, for entrance into Kindergarten and 1st (or date of the disease).
Exceptions, in the absence of an epidemic or immediate threat thereof, will
be granted to any student whose parent shall file with school authorities a
signed, written statement that such measures conflict with his religious
tenets and practices; or any student due to medical reasons, if such student
has a written statement from his doctor excusing him form such immunization.
Proof of exceptions will be in writing and filed in the same manner as other
immunization records.
