Optional Healthy Lifestyle Wellness Plan

Optional Healthy Lifestyle Wellness Plan is for Employees not assigned to Wellness Plan Level 1 or 2. Participants that complete at least 4 points and enter their activities in AHA are eligible for prize drawings.  Prize winners must provide documentation for completed points.

  • Eligible Dates July 1-June 28, 2019.
  • Participants must complete all activities by June 28, 2019.

Activities Eligible for Points / Point Value

  • Annual physical with PCP (Primary Care Provider) / 1 Point
  • Active participation and compliance in the “Diabetes Care Program” / 1 Point
  • Dental Screening, Bi-Annual (1 point each for up to 2 cleanings per year) / 1 Point
  • Preventative Dermatologist exam for skin cancer / 1 Point
  • Annual Flu Shot / 1 Point
  • Colonoscopy (as recommended by PCP) / 2 Points
  • Cardio exercise / 1 Point
    • 1 point for each 4 week exercise log completed
    • Minimum of 150 min/week for 4 weeks required
  • Tobacco cessation (at least 4 weeks) / 3 Points
    • Must discuss with Program Office
  • Food log for 1 week tracking specific foods as set with Health Coach / 1 Point
    • 1 point for each 7-day log
  • Health Coaching session with nurse or dietician / 1 Point
    • face-to-face or by telephone
    • Must be actively working on specific goals
    • 1 point for each session - at least 30 days apart
  • 1 point for each 5% or 10 lbs. of body weight loss from previous documented weight/ 1 Point
    • Must be weighed in Program Office & documented
  • “Special Programs” as approved by Program Office. / 1 Point
    • Examples: Turkey Trot, Bluegrass Marathon, Girls on the Run, etc.
  • Educational Class on topics that pertain to individual and goals to improve health. / 1 Point
    • 1 point for each hour of class
  • Mammogram / 1 Point
  • Vision exam (dilated exam) / 1 Point
  • Pelvic/Pap Smear / 1 Point
  • PSA lab and digital exam / 1 Point
